It was introduced in August 2020 to support UK health and social care sector recruitment and offers significant advantages over the standard Skilled Worker route: reduced salary thresholds, full exemption from the Immigration Health Surcharge (IHS) for both the main applicant and their dependants, and priority processing. After five years of continuous qualifying residence, Health and Care Worker visa holders may apply for Indefinite Leave to Remain.
The visa applies to qualified healthcare professionals working for the NHS, NHS-contracted organisations, and CQC-registered adult social care providers. From 22 July 2025, the route underwent a significant change with the closure of the care worker occupations to new overseas applicants — a critical development that all NHS HR teams and adult social care organisations need to understand.
From 22 July 2025, the Health and Care Worker Visa is CLOSED to new overseas recruitment in the care worker (SOC 6135) and senior care worker (SOC 6136) occupation codes. UK employers — including CQC-registered adult social care providers — can no longer issue Certificates of Sponsorship for new overseas hires into these roles.

Those roles would need to be sponsored under the standard Skilled Worker Visa route if they meet the general eligibility criteria. From 8 January 2026, applicants must meet the English language requirement at CEFR Level B2 for new applications. This can be demonstrated through an approved Secure English Language Test, a degree taught in English, or on the basis of nationality. Many NHS roles have their own professional registration requirements — for example, nurses applying for NMC registration must pass the OSCE and competency-based test, which also demonstrates English language proficiency.
Health and Care Worker Visa holders in eligible clinical roles may bring a partner and dependent children under 18 to the UK. Both the main applicant and their dependants are exempt from the Immigration Health Surcharge. Care workers (SOC 6135/6136) transitioning under the 2025 to 2028 arrangements may not bring new dependants. Dependants already in the UK before the relevant date may remain under existing leave.

No. From 22 July 2025, care workers (SOC 6135) and senior care workers (SOC 6136) can no longer be sponsored under new Skilled Worker Certificates of Sponsorship for overseas applicants. This category is closed to new international recruitment. Existing overseas care workers who held valid Skilled Worker leave before this date may extend their visa under transitional arrangements until 22 July 2028, but new overseas applicants for these specific roles cannot apply.
Qualified clinical professionals whose roles are on the Skilled Worker eligible occupation list and who work for a qualifying employer remain eligible. This includes registered nurses and midwives, doctors and consultants (all grades), physiotherapists, occupational therapists, speech and language therapists, radiographers, paramedics, psychologists, pharmacists, dental practitioners, and many other Allied Health Professions.
Three specific financial advantages: (1) reduced visa application fee — approximately half the standard Skilled Worker fee; (2) full exemption from the Immigration Health Surcharge, saving approximately £624 per year per person (and for dependants); and (3) priority visa processing is available at no additional cost, compared to the additional fee charged on other routes.
For most adult social care roles outside the NHS, the sponsoring employer must be registered with the Care Quality Commission (CQC) as a provider of regulated activities. NHS trusts and NHS-contracted healthcare providers are exempt from the CQC registration requirement because they are regulated differently. For adult social care employers, CQC registration at the time of sponsorship is a mandatory prerequisite.
The Health and Care Worker Visa uses lower thresholds than the standard Skilled Worker route. For NHS Agenda for Change roles, the relevant pay band going rate applies — Band 3 is £25,760 from April 2026. For non-NHS healthcare roles, the applicable minimum is the higher of £25,000 or the going rate for the occupation. Clinical management roles must meet the higher of £31,300 or the going rate.
Yes, for most roles. From 11 March 2024, care workers (SOC 6135) and senior care workers (SOC 6136) who applied on or after that date cannot bring new dependent family members.
